The (Assistant) Nurse Clinician (Dementia Care) supports the delivery of person-centred, evidence-informed nursing care for older adults living with dementia across St Luke's Elder Care's centre-based, community-based, home-based, and residential care settings. The role provides clinical assessment, care planning, caregiver education, and team support to enhance the safety, dignity, functional ability, and quality of life of seniors with dementia, in alignment with St Luke's Elder Care's philosophy of care and whole-person approach spanning physical, psycho-social, cognitive, and spiritual domains.

Key Responsibilities:

  • Conduct comprehensive nursing assessment for seniors with dementia, including physical health, cognitive status, functional ability, behavioural and psychological symptoms, fall risk, nutrition, skin condition, and medication-related concerns.
  • Develop, implement, and evaluate individualized care plans for persons living with dementia using person-centred care principles and appropriate risk management measures.
  • Deliver direct nursing care, monitor clinical condition, identify deterioration early and escape to the doctor, Nurse Manager, or relevant multidisciplinary team members when required.
  • Support the management of common behavioural and psychological symptoms of dementia through non-pharmacological approaches, structured routines, therapeutic communication, and de-escalation strategies.
  • Apply effective communication techniques when engaging persons with dementia, family members and caregivers to promote cooperation, reassurance, and meaningful participation in care.
  • Educate and support caregivers on dementia condition awareness, daily care techniques, behaviour management, safety supervision, self‑care, and available community resources.
  • Collaborate with doctors, allied health professionals, care staff, and centre teams to ensure coordinated, holistic and continuous care for clients with dementia.
  • Maintain timely and accurate clinical documentation, nursing notes, care plans, incident reports and follow-up actions in accordance with organisational and regulatory standards.
  • Participate in case discussions, quality improvement activities, clinical audits, and service development initiatives related to geriatric and dementia care.
  • Serve as a clinical resource to junior nurses and care staff by reinforcing good dementia care practices, infection prevention, safe care processes and respectful client handling.
  • Support and conduct training, orientation and competency development for staff involved in caring for persons with dementia.
  • Support or develop programmes for persons living with dementia in collaboration with Programme Development Unit.
  • Participate in committees or workgroup as assigned.
  • Uphold compliance with St Luke's Elder Care policies, Singapore Nursing Board standards, infection control requirements and relevant community care regulations.

Qualifications &

Experience:

  • Registered Nurse with a valid practising certificate recognised by the Singapore Nursing Board.
  • Degree in Nursing or equivalent recognised nursing qualification. Post‑basic training in gerontology, community nursing, dementia care or related discipline will be an advantage.
  • Minimum 3 to 5 years of nursing experience, preferably in community care, geriatric care, rehabilitation, day care, home care, or nursing home settings.
  • Prior hands‑on experience caring for seniors with dementia, including managing behavioural and psychological symptoms of dementia, is strongly preferred.
  • Good clinical assessment, care planning and documentation skills.
  • Ability to work effectively with multidisciplinary teams and engage families and caregivers with empathy and professionalism.
  • Strong communication, problem‑solving and organisational skills, with a practical and hands‑on approach to care delivery.
  • Passionate about caring for persons with dementia.
  • Comfortable working in an operational healthcare environment and travelling across service settings where required.
