This permanent role has a starting salary of £34,660 per annum based on a 36-hour working week.

We have a great opportunity to join our Connect to Community (C2C) West Mental Health and LD&A team as a Senior Social Care Assistant. We have offices at both Victoria Gate in Woking and Millmead in Guildford and you will be able to indicate your preferred office as part of the application process.

We support hybrid working with the right balance. We come together in person for 2 days per week on average (40% of the working week) and support working from home the rest of the time.

About the Service

Connect to Community is Surrey's single front door for adult social care, supporting residents through timely, strengths-based responses following initial triage. C2C brings together professionals from different disciplines to ensure residents receive the right support, at the right time, with fewer handovers.

Senior Social Care Assistants in C2C play a critical role in:

Supporting service users to be able to be in more control of their care, making informed choices and being active citizens in their community
Supporting consistent, high-quality practice across the team.
Working closely with partners across health, housing, reablement, TEC and the voluntary sector
Daily multi‑disciplinary meetings provide a strong forum for shared learning, professional challenge and collaborative risk management.
About the Role

Connect to Community is a fast-paced, front-door environment where, as a Senior Social Care Assistant, you will have frequent direct contact with the public, both over the telephone and through visits to individuals in their own homes or care settings.

In this role, you will undertake assessments, reviews and targeted interventions in line with the Care Act, alongside taking on safeguarding responsibilities, including Section 42 enquiries. You will promote a recovery-focused, strengths-based approach, supporting individuals to achieve positive outcomes and maximise their independence. The role requires the application of sound professional judgement to manage risk, uncertainty and competing priorities effectively, whilst developing and implementing strengths-based support plans that enhance wellbeing, choice and control.

You will also work collaboratively with individuals, carers, multidisciplinary professionals and service providers to ensure coordinated and effective support for vulnerable adults and their families.

Your Application

In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours:

Working knowledge of the Care Act legislation and how it impacts the work we do
Substantial experience of and confidence working with residents with a variety of physical and cognitive social care needs
Competence in a range of IT skills and a willingness to learn to use a variety of software systems.
Problem solving skills
Excellent communication skills, with the ability to convey information clearly, concisely, and respectfully, both verbally and in writing.
A commitment to your own professional development
The ability to empathise with, and show compassion towards, individuals in need of social care assistance.
