In this role, you will assess, plan and deliver occupational therapy interventions for adults with physical, functional and long-term conditions living in the community. You will support people to remain safe and independent at home this includes utilising equipment, moving and handling advice, and recommendations for minor and major adaptations. Working closely with colleagues across health, housing and social care, you will manage a varied caseload and contribute to high-quality, strengths-based practice.
You will:
- Carry out holistic assessments and identify practical solutions to support independence and safety.
- Recommend and review equipment, moving and handling solutions, and housing adaptations.
- Manage a defined caseload, prioritising risk and need while maintaining accurate records.
- Work collaboratively with people, their carers and partner professionals to deliver coordinated support.
- Support junior staff and contribute to service development, learning and continuous improvement.

We are a small team doing big things and making a big impact. You would be joining a well‑established team where respect, team working and a supportive environment are key to a successful future. We operate a flexible way of working over the week, you will manage your own diary and there is no weekend working. We are mostly agile and work from home, whilst using the office as an official work base.
Our wider Occupational Therapy service supports adults and children across areas such as moving and handling, adaptations, Technology Enabled Care, Learning Disability, and OT Duty triage. This post sits within our Adults Occupational Therapy team, delivering assessment and intervention within the home environment.
You will be a qualified and HCPC‑registered Occupational Therapist with a strong commitment to person‑centred practice. You will bring sound professional judgement, excellent communication skills and the ability to work independently while contributing positively to a team. You will be motivated by making a difference, confident in managing complexity, and committed to your own professional development. You will have the following:
- Degree in Occupational Therapy and current HCPC registration.
- Experience of working in health or social care, including complex cases and multidisciplinary working.
- Knowledge of relevant legislation, including the Care Act, housing and moving and handling practice.
- Strong assessment, problem‑solving and risk management skills.
- Ability to build effective relationships with people, families, carers and professionals.
- Commitment to high‑quality record keeping, learning and continuous professional development.
Please note this role is based in the community, it is essential that you hold a full driving licence and have access to a car to complete home visits.
