Position Purpose

Leads the strategic execution and operations of the research portfolio that underpins three service arms of Dartmouth Health @ Home (Primary Care at Home, Palliative Care at Home, General Home Care). The role ensures rigorous study design, rapid‑cycle evaluation, and seamless integration of faculty expertise with operational teams. It also cultivates and manages relationships with internal and external partners, including researchers, the operations team, NH/VT Medicaid agencies, the Centers for Medicare and Medicaid Services, rural health transformation leaders, and donors, to generate evidence to inform sustainable payment and financing models for home-based care.

Job Description

Dartmouth Health at Home is an emerging research and innovation program dedicated to designing, implementing, and evaluating sustainable home-based models of care that improve caregiver experience and patient outcomes. Building on Dartmouth’s national leadership in rural health, population health, and healthcare delivery innovation, this initiative addresses critical challenges in rural regions, including caregiver preparedness, workforce capacity, geographic distance, and socioeconomic and insurance heterogeneity.

The Dartmouth Institute for Health Policy & Clinical Practice ( TDI ) plays a central role in advancing the program’s research mission, offering opportunities to engage in implementation science, health economics, and health policy research. Team members will contribute to the design and evaluation of innovative care models, generate generalizable evidence, and help inform reimbursement strategies and policy development at the regional and national levels.

This is a unique opportunity to be part of a “learning health system” environment where care delivery and research are tightly aligned, enabling individuals to shape scalable, evidence-based solutions that improve care for rural and underserved populations.

Co‑led by Dartmouth Health, the program brings together clinical care delivery, population health expertise, and TDI’s interdisciplinary research strengths. Faculty and staff engaged in this work will collaborate across disciplines in a highly integrated academic health system.
