Job SummaryThe Primary Health Care Manager, as a part of the Primary Health Care & Chronic Disease Management team, is responsible and accountable for the provision of efficient, effective and quality Primary Health Care services within the defined geographic area of Bell Island and the surrounding area (urban) of Eastern Health. The incumbent will work across multiple primary health care sites with primary responsibility for the Dr.
Walter Templeman Health Centre. The Dr. Walter Templeman Health Centre is a rural health centre located on Bell Island and includes primary health care services, long-term care and emergency services. The incumbent will perform the role of site manager for the Dr. Walter Templeman Health Centre working across programs.
The Site Manager provides leadership in the planning, implementation and evaluation of an integrated team-based organizational structure and service delivery model including a focus on primary health care which will respond to the needs identified in the Bell Island Community Needs Assessment and through the Community Advisory Committee (CAC). The incumbent will maintain and evaluate programs and services to ensure quality care is provided to the residents of Bell Island.
The incumbent will have primary responsibility for the day-to-day operation of the facility and will provide leadership to the staff of the facility. The Site Manager will also be responsible for guidance and advice to those individuals that do not report directly to the manager. This position will work in collaboration with other managers for quality reporting and in the provision of support systems related to the overall operation of the Health Care.
The incumbent will maintain strong working relationships with Eastern Health departments and programs, other service providers and the community.
The Manager is responsible for the planning, development, implementation & evaluation of the Primary Health Care services.
Job Qualifications Education- A Baccalaureate degree in a relevant health services discipline is required.
- Master's degree is preferred.
- Minimum of 5 years' experience within the last 7 working in Acute, Long Term Care or Community care is required.
- Considerable experience in a leadership role within Acute, Long-Term Care or Community is required.
- Experience in leading collaborative teams is considered an asset.
- Demonstrated knowledge and proficiency in the Microsoft Office Suite of applications.
- Demonstrated excellent interpersonal, organizational and leadership skills.
- Demonstrated exceptional oral and written communication skills.
- Demonstrated strong analytical, facilitation and negotiation skills together with sound judgment, diplomacy.
- Demonstrated ability to establish and maintain respectful working relationships with clients/patients/residents, staff, families and the community.
- Demonstrated knowledge and experience in organizational management, client centered care and working with communities.
- A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
- A satisfactory record of work performance and attendance is required.
