Salary:
Grade 5, £18,124 - £19,959 per annum. £13.90 - £15.31 per hour
3 positions x 25 hours per week

Your roleThe role is a deeply rewarding position within the team that supports residents of the borough back into their own homes.
As a Reablement Support Worker you will be part of the team providing integrated, multidisciplinary programmes of Reablement across the borough.
We are part of Early Intervention which is a multidisciplinary team consisting of Reablement, Therapists, Social Care Workers, Housing and Health professionals.
The Reablement service operates 7 days a week, 365 days a year to include all Bank Holidays operating between the hours 07:00 through to 22:00.
This role can exceed normal hours of work in line with the service needs.
You will work every other weekend between the hours 08:00 through to 22:00.
About youYou must be creative and self-motivating with a strong customer care ethic. You will be able to demonstrate good interpersonal and communication skills and the value of promoting independence.
You will be providing support from the Reablement service in the person’s home. You will use an asset-based approach to promote independence, helping people to remain within their own homes and also connect within their communities to enhance their well‑being.
This post will require an enhanced DBS.
You will be required to hold a current valid driving licence and have access to a vehicle giving you the ability to travel as required to meet the needs of the service.

The team will support people who have just left hospital or following significant changes in a person’s ability to thrive will support people to regain the confidence, ability and skills to live at home.
