Your responsibilities will include undertaking Care Act assessments, coordinating discharge planning, managing complex cases, and ensuring that appropriate care and support packages are in place to promote safe recovery and independence in the community.
QualificationsApplicants must hold a recognised Social Work qualification and be registered with Social Work England (SWE). Previous experience in hospital discharge, safeguarding work, and integrated health and social care settings is essential. Experience using Mosaic or similar case management systems is beneficial.
