Chronic Care Staffing is currently hiring a highly organized and detail-oriented Scheduling Coordinator to support its patient referral and care coordination operations. This is a fully remote position based in the United States, offering the flexibility to work from anywhere while contributing to essential healthcare processes.

Responsibilities
  • Review electronic health records (EHRs) to gather and verify patient information.
  • Retrieve and organize essential documentation such as demographic sheets, insurance cards, and office visit notes.
  • Prepare and submit referral packets to external medical specialists.
  • Process referrals through fax, phone, and secure healthcare portals.
  • Identify appropriate referral destinations using internal specialty resource guides.
  • Ensure referral packages are complete, accurate, and compliant with standards.
  • Collaborate with follow-up and care coordination teams.
  • Cross-train on additional workflows to support operations.
  • Monitor documentation quality and correct missing or incorrect information.
Requirements
  • 1–2 years of experience in medical office administration or healthcare support roles.
  • Comfortable working with Electronic Health Records (EHR) systems.
  • Strong computer and data entry skills.
  • Excellent attention to detail and accuracy.
  • Ability to manage multiple tasks and prioritize workload.
  • Strong written and verbal communication skills.
  • Ability to work independently in a remote environment.
  • Strong knowledge of healthcare administration processes.
  • Strong knowledge of patient referral coordination.
  • Strong knowledge of medical documentation systems.
