Elevate patient care as a Field Health Case Manager with Sentrex Health Solutions, a remote/hybrid role focused on patient support and program coordination in Québec. Engage with healthcare professionals to enhance treatment access and delivery.
In this full-time position, you will manage patient support programs, provide necessary training, and utilize PSP applications effectively. Your goal is to ensure timely and quality services while coordinating efforts between patients, clinics, and program managers.
Key Responsibilities:
• Oversee patient enrollment and program access
• Provide training on patient support services
• Complete and process reimbursement and enrollment documentation
• Manage clinic visits and dose reconciliation efforts
• Recommend improvements to enhance patient outcomes
Requirements:
• Post-secondary education is necessary
• Bilingual in English and French
• 3–5 years in reimbursement or patient support
• Customer-service focused with problem-solving skills
• Ability to travel and maintain a designated remote workspace
