About the role
As a Feel Good Suffolk Advisor in the Communities Team
, you will provide holistic health and wellbeing support to clients accessing the FGS service. You will primarily be based in the community with some remote/office working, travelling across the district to provide face-to‑face services and client support over the phone.
Key responsibilities:
- Provide triage and assessment of clients accessing the FGS service.
- Deliver behaviour change interventions and techniques based around Smoking Cessation, Physical Activity and Weight Management as required.
- Motivate and empower clients to make behaviour changes, conducting assessments and creating personal action plans to improve health.
- Use Health Coaching techniques to deliver face‑to‑face, digital and telephone 1:1 and group interventions.
- Develop and co‑produce action plans to provide personalised support to individuals, families and carers.
- Deliver group and one‑to‑one smoking cessation support, including coordinating Nicotine Replacement Therapy (NRT) and using a CBT approach.
- Work closely with partners at place and neighbourhood level to refer and signpost clients to suitable support, including local authorities, integrated care board and primary/secondary care, leisure providers and VCFSE organisations.
- Complete ‘touch point’ assessments and evaluations, recording data in a case management system and reporting on outcomes.
- Contribute to the measurement of change and improvement in health behaviours using relevant outcome measures.
- Equip people with knowledge, skills and confidence to access information and support via a digital front door.
- Collaborate with colleagues across the county to ensure efficient working.
- Attend community events and complete outreach within the districts.
About you
- Passionate and motivational, with experience in local government, health or voluntary sectors supporting community health and wellbeing.
- Good understanding of community health issues and the strengths and barriers faced by residents.
- Caring nature, empathy and understanding to support individuals from diverse backgrounds and varying support needs.
You will need to demonstrate:
- Understanding of barriers to healthy lifestyles and health inequalities.
- Good knowledge of the local area and health provision.
- Understanding of referral, triage and assessment processes.
- Experience using a case management/customer relationship management system.
- Experience building personalised action plans and working towards KPIs and evaluation processes.
- A broad range of knowledge on health and wellbeing issues.
- Ability to manage working relationships and confidently network and make new contacts.
- Desire to undertake training and development opportunities to ensure continual professional development.
A full UK driving licence and access to a car is essential for this role.
