Dietitian

Position: Dietitian Role at CBI Home Health
Support patient health as a Registered Dietitian with CBI Home Health in the Durham region. This role involves delivering home-based nutrition support.

In your pivotal position in the Integrated Care Program, you’ll assist patients in their transition from hospital to home. Utilizing your passion for nutrition and excellent communication, you will work independently and with an interdisciplinary team to enhance patients’ quality of life.

Key Responsibilities:

• Provide direct nutrition support to patients at home

• Collaborate with healthcare team members effectively

• Contribute to patient education about nutrition

• Utilize tube feeds if necessary for care plans

• Focus on preventing patient readmissions through nutrition

Requirements:

• Must be registered with the College of Dietitians of Ontario

• Experience with tube feeding is a welcomed asset

• Empathetic and passionate regarding patient care

• Strong communication and teamwork skills

• Bilingual abilities in English and French are advantageous
