£37.34 per hour Umbrella
37 hours per week
Initial 3-month contract
Hoop Social Work is recruiting for an experienced Qualified Social Worker to join the Hospital Team based at St Woolos Hospital in Newport. This is a fantastic opportunity to work within a supportive, fast-paced multidisciplinary team, with a hybrid working arrangement combining time on-site and working from home.
As a Hospital Social Worker, you will play a key role in supporting timely, safe hospital discharges by completing strength-based assessments and coordinating appropriate care and support plans. You will work closely with health professionals, patients and families to achieve positive outcomes while promoting independence.
Key Responsibilities
Complete assessments in accordance with the Social Services and Well-being (Wales) Act.
Undertake hospital discharge planning in a busy acute setting.
Apply the principles of the Mental Capacity Act where appropriate.
Identify potential Continuing Healthcare (CHC) triggers and contribute to the assessment process.
Work collaboratively within a multidisciplinary team to support effective discharge planning.
Complete person-centred, outcome-focused assessments and care planning.
Liaise with a range of statutory and voluntary agencies to ensure seamless transitions from hospital.
About You
To be considered for this role, you will need:
A recognised Social Work qualification.
Registration with Social Care Wales or Social Work England but we willing to register with SCW ahead of starting
Experience working within adult services, ideally within a hospital or discharge setting.
Knowledge of the Social Services and Well-being (Wales) Act.
Confidence in applying the Mental Capacity Act.
Excellent assessment, communication and partnership working skills.
An Enhanced DBS (registered on the Update Service or willingness to apply).
Experience using the WCCIS system would be advantageous but is not essential.

Initial 3 month contract with the possibility of extension.
