Hoop Social Work is looking for an experienced Qualified Social Worker to join their Hospital Team at St Woolos Hospital in Newport. This role offers hybrid working, allowing a balance of on-site and home working, and involves supporting safe hospital discharges through comprehensive assessments and care plans.
The ideal candidate will have a recognised Social Work qualification, relevant experience, and excellent communication skills. This position also includes an initial 3-month contract with the potential for extension.
