Salary: £39,959 – £48,117 per annum (pro‑rata)

Visa Sponsorship

We provide visa sponsorship to applicants who require this in order to have the right to work in the UK, provided the role you have been offered meets the requirements set by the Home Office. Applicants requiring a Skilled Worker Visa (e.g. data analysts, IT and finance professionals) will need to meet the salary threshold of £41,700. Job Overview

Are you looking to further your skills as an excellent patient‑focused holistic clinician? Do you want to be part of an exciting team that bridges the gap between the acute hospital and community setting? Do you want to make a difference to a patient’s outcome? If so, our Central Locality team may be the right fit for you. We work closely with multidisciplinary teams, including physiotherapists, occupational therapists, assistant practitioners and rehabilitation support workers, nurses and paramedics, to promote independence through rehabilitation and crisis management with patients in their own homes or short‑term care settings.

Duties

You will need excellent assessment and intervention skills to plan, develop and evaluate goal‑focused rehabilitation and reablement programmes, and complex discharge planning experience. Understanding of health and social care provision and the ability to work closely with other organisations and stakeholders is essential. You must demonstrate high standards of evidence‑based clinical practice, excellent interpersonal and communication skills, the ability to prioritise your workload, manage your time effectively and work flexibly within the team.

Qualifications

and Experience Training and Qualifications
  • Diploma/degree in physiotherapy
  • Current registration with HCPC
  • Evidence of core rotations (MSK, orthopaedics, neurology, respiratory, care of the older adult)
  • Evidence of specialist post‑graduate training and education with older adults
Experience
  • Significant post‑graduate physiotherapy experience in a variety of clinical settings, including a community setting
  • Experience working in multidisciplinary/multi‑agency teams
Knowledge and Skills
  • Ongoing awareness of recent developments in practice and research, including NSF’s/NICE guidelines
  • Understanding of clinical governance and risk management and their implications for the service
  • Effective communication within the team
Other
  • Must be able to meet the travel requirements of the post

You must have appropriate UK professional registration.
