VHA Home Health Care is seeking a dedicated Dietitian to provide home-based nutrition assessments and personalized consultations for children and adults. This role requires a compassionate approach to cater to diverse client needs. You will create tailored diet plans, educate clients, and maintain communication within healthcare teams.

Key Responsibilities:

• Create tailored diet plans for clients and caregivers

• Prepare client assessments and reports for healthcare teams

• Educate clients on nutrition and health topics

• Address dietary issues related to medical concerns

• Ensure quality improvement in client care

Requirements:

• Registered with the College of Dietitians of Ontario

• Minimum of 2 years clinical practice experience

• Valid Driver's License and access to transportation

• Compassionate nature with a strong work ethic

• Skilled in documentation and caseload management
