Leeds and York Partnership NHS Foundation Trust (LYPFT) offers a range of mental health and learning disability services. We are seeking a motivated and enthusiastic Care Coordinator to work in the Working Age Community Mental Health Service (CMHT). The role supports service users’ recovery within the community and is part of the broadband transformation programme designed to improve community mental health care for people with severe and enduring mental illness.

As a Band 6 Care Coordinator you will:

  • Conduct initial assessments, care coordination and team duty
  • Develop and deliver complex service‑user‑focused care plans and formulate needs in partnership with the multidisciplinary team
  • Provide clinical supervision, preceptorship and support to junior colleagues and student education
  • Exercise leadership within your role and contribute to continuous improvement of service delivery
  • Apply risk assessment skills and a range of evidence‑based therapeutic interventions
Responsibilities
  • Coordinate continuous care for service users across the service and where necessary with wider services
  • Negotiate workload and caseload through supervision and work‑load management with clinical team managers
  • Maintain service user recovery goals as central to formulation, care planning and review
  • Support the team with leadership, clinical supervision and preceptorship activities as part of your role
  • Implement safe risk management and intervene appropriately in the event of escalating risk or emergencies
Qualifications
  • Relevant core professional qualification: RMN/RNLD/OT/SW (or equivalent)
  • Full UK driving licence and access to own vehicle (where required)
Experience
  • Experience working with people with ongoing mental health needs
  • Experience working with people at risk
  • Demonstrated experience formulating care plans for patients

  • Full UK driving licence and access to own vehicle
Hours & Location
  • Full time or part time (pro rata for part time)
  • Hours:

    37.5 hrs per week (temporary part‑time 22.5 hrs also available)
  • Site: CMHT Adult West (St Mary’s Hospital) or South (Aire Court), Leeds
Salary

£39,959 – £48,117 per annum (pro rata for part time). Salary is graded at Band 6.
