Position: Home-Based Physiotherapist at The Care Company

This role offers an opportunity for experienced physiotherapy professionals who value autonomy and client relationships. The ideal candidate will focus on individualized treatment plans, mobility training, and educating clients and caregivers. Your mission is to support clients of all ages in safely thriving at home, as you collaborate with a multidisciplinary care team.

Key Responsibilities:

• Provide follow-up sessions for mobility and pain management

• Develop and adjust individualized treatment plans

• Educate clients on exercise programs and safety strategies

• Identify fall risks and environmental barriers

• Collaborate with OTs, PSWs, and nurses for coordinated care

Requirements:

• Minimum 2 years of experience in home care

• Registered with the College of Physiotherapists of Ontario

• Strong documentation and communication skills

• Valid driver's license and reliable transportation

• Comfortable working independently in community settings
