HCRG Care Group is recruiting a Band 6 Physiotherapist to join the Early Supported Discharge team at Gravesham Community Hospital, Gravesend. You will deliver high-quality neuro-rehabilitation in patients' homes, working with a multidisciplinary team to support safe discharge and ongoing recovery.
Essential experience includes neurology or community-based rehabilitation, HCPC registration and the ability to work independently within a collaborative team.
