Overview
Cwm Taf Morgannwg University Health Board is seeking an Occupational Therapist Specialist for a static Band 6 role within the Rapid Assessment and Prevention OT service.
The post supports rapid, multi-factorial assessments and admission avoidance, with the primary base at the Clinical Navigation Hub (Ty Elai). OTs will rotate between hub-based and roaming roles across CTMUHB and Local Authority sites to reduce unnecessary travel, with a focus on keeping individuals in their own homes.
The service works collaboratively with GP and Welsh Ambulance colleagues to intervene at the earliest point of crisis.

Main duties of the jobThe primary focus will be rapid multi-factorial assessments in individuals’ homes to identify what matters to them, co-produce achievable goals, and implement fast-paced interventions.
The post-holder will work collaboratively with integrated Health and Local Authority community teams, delivering on a place-based care ethos. You will prioritise and screen referrals with the aim of offering a same-day response. This will include telephone consultation and home-based assessment, with scope to complete up to two additional sessions to support individuals to return home during acute need. Your scope of practice will include compensatory and functional rehabilitation methods to prevent admission and support the person to manage immediate needs.
You will form close working relationships with integrated primary care and rehabilitation colleagues to hand over the episode of care and support longer-term goals.
You will be part of the service design and will work closely with the Band 7 Clinical Lead, Band 6 OT colleague, and Principal Occupational Therapist to implement change at pace.
The ability to speak Welsh is desirable;

Job responsibilitiesYou will be joining a well-established Network of integrated community and primary care with a dedicated Principal Occupational Therapist for professional support and leadership. The post holder will work closely with the wider multi-disciplinary teams to meet the needs of people within their communities. The post will offer opportunities to:
