Position: Registered Dietitian Hybrid Role at SE Health
Transform patient care as a Registered Dietitian at SE Health. This hybrid role allows you to provide client-centered nutrition support in both community-based and rehabilitation settings.

SE Health is seeking passionate Registered Dietitians to join their Acute Care Transitions Program. You will work closely with a diverse interdisciplinary team to create holistic care plans tailored to each client's unique needs. Your responsibilities will include conducting assessments, formulating treatment goals, and managing community resources to ensure comprehensive care delivery.

Key Responsibilities:

• Conduct client assessments to understand needs and conditions

• Formulate personalized goals and professional recommendations

• Develop and implement client-centered treatment programs

• Collaborate effectively with interdisciplinary rehabilitation teams

• Adapt to hybrid work settings, including remote and in-person care

Requirements:

• Experience in community or acute care settings preferred

• Reliable, creative, and self-directed with strong communication skills

• Knowledge of community resources supporting patient recovery

• Vehicle access and valid driver’s license required

• Positive work ethic and attendance record essential
