The NHS in Sherborne is seeking a Health Care Practitioner specialising in Older People to join our Frailty service. You will work with the MDT to identify and support comprehensive geriatric assessments and personalised care plans for elderly patients, enabling independent living at home.
You will deliver care in patients' homes, the Grove Medical Centre, and potentially The Apples Medical Centre and Yetminster Health Centre, with part-time hours and home-based outreach.
