Derbyshire Healthcare NHS Foundation Trust is seeking a Lead Nurse in the Dementia Rapid Response Team to deliver compassionate, non-pharmacological care for older adults with dementia at risk of crisis. Based in Bakewell, you will lead a MDT, assess needs, and coordinate home-based interventions to prevent hospital admission and support early discharge when appropriate.
The post requires RMN with community and dementia experience, and offers a permanent full-time role with a 365-day service
