In this pivotal role, you will work within Carewell's innovative care navigation program focused on proactive patient outreach. Your responsibilities include managing an outbound referral queue, guiding patients through intake processes, and using empathetic communication to address any hesitations they may have about care. This role thrives on metrics, looking for someone who drives patient follow-through and demonstrates adaptability in a fast-changing environment.
Key Responsibilities:
• Conduct proactive telephonic outreach with prospective patients
• Complete clinical intake questionnaires accurately in real-time
• Build rapport with older adults to navigate their concerns
• Adhere to compliance protocols for verbal consents
• Schedule appointments immediately following successful intake
Requirements:
• 1+ years of patient-facing healthcare experience
• 1+ years in sales or program enrollment
• Proven experience in outbound phone environments
• Comfortable with multiple software systems
• Reside in Alberta, Canada, and legally authorized to work
