Location: Eling
Hampshire County Council is seeking a Reablement Practitioner to work across disciplines, supporting people to live at home and promote independence.
You will triage referrals, carry out holistic assessments, and plan interventions with a strengths-based approach, building relationships with service users and professionals. Hybrid working at Totton Hub, permanent contract, with a salary of £33,178–£36,369 per annum.
