Rochdale Borough Council is seeking an Adults Social Worker to join the Hospital Discharge team in Rochdale. You will assess and support adults and carers, ensuring safe and timely discharges from hospital sites.
You will work with health colleagues, use ICT, maintain records, and may have some home-based hours. A driving licence and access to a car are essential, with site visits to local Rochdale hospital sites.
