RDaSH NHS Foundation Trust in Doncaster is recruiting an Acquired Brain Injury Specialist Case Manager to lead complex ABI case coordination across South Yorkshire, working with people with ABI, families, health, social care and partner agencies.
You will assess needs, develop care and risk management plans, and support repatriation to reduce out-of-area placements while improving local pathways and patient flow.
