We always strive to get people back home through a strengths-based and person-centred approach, and support them to live their best lives.
We are a hard-working team based in a fast-paced environment, regularly going up to the wards to meet our clients or visiting them at home after they've been discharged. We work closely with our health colleagues, ensuring a multi-disciplinary approach to discharge planning. There is a supportive culture embedded within the team, and staff step in to assist their colleagues to ensure discharges from hospital are timely.
As a team, we are committed to making a difference to our residents lives, ensuring no one is left behind, and working in a positive and inclusive manner.
