Leeds Community Healthcare NHS Trust is offering a Band 5 Pharmacy Technician to join the Ageing Well Team within Woodsley and Holt Park PCN in North Leeds. You will work with a multidisciplinary team delivering medicines-related care across ten GP practices, including home visits and medication reviews under supervision of senior pharmacy staff.
You will support medication management, adherence, and home-based strategies to optimise outcomes.
