As part of the dedicated EMP team, you will execute essential dietary assessments and coordinate nutritional support for home-based patients.

Your role is crucial in promoting patient well-being and increasing satisfaction levels through personalized care plans. Work Monday to Friday while engaging with a variety of community services.

Key Responsibilities:

• Conduct nutritional assessments of patients at home

• Formulate and manage individualized dietary plans

• Educate and empower patients and families on nutrition

• Collaborate with interdisciplinary team members

• Uphold confidentiality and professional standards

Requirements:

• Bachelor Degree in Nutrition Sciences or equivalent

• Registered with the New Brunswick Dietitians Association

• Valid Class 5 Driver's License and good driving record

• CPR Level C certification or willingness to obtain

• Proven ability to work as part of a team
