Advanced clinical practitioners will work across the four pillars of advanced practice; clinical, leadership, research and education and cover a range of services including, but not limited to: ABC Remote Services (uCATS, Enhanced Access, Additional Primary Care, Smoking Cessation and ADHD Primary Care Connect)).

As the post holder, you will autonomously undertake assessment and treatment of both acute and chronic, differentiated and undifferentiated, patient presentations.

You will determine a clinical diagnosis, treatment plan and appropriate discharge, or refer, as indicated whilst maintaining accurate patient records and ensuring patient safety.

You will also provide clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team.

At all times, you will ensure a high-quality patient-centred service is delivered in remote settings across all clinics.

Regular supervisions on a one-to-one basis including a annual formal appraisal. If you are in a patient-facing role, you will also receive regular Clinical Supervisions with a Clinical Supervisor. There are regular clinical service training days and opportunities for CPD

As the successful applicant, you will be an experienced healthcare practitioner within acute and urgent care. Ideally, you will have completed an MSc in Advanced Practice, be independent prescribing, and be able to evidence a clinical portfolio and relevant competencies in line with the HEE national framework for advanced clinical practice.

Full time/part time service coverage 08.00 to 20.00 Monday to Sunday

the role is remote based with monthly visits to our offices in Horley, Surrey and/or Crawley, West Sussex and is available on a full or part-time basis.

• To independently undertake specialised clinical assessments, diagnose, treat and discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
• To ensure patients are fully involved in the design and implementation of their treatment plans.
• To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where urgent care is required.
• To refer appropriately to clinical leads/supervising GP where a second opinion is required.
• To adapt styles of communication appropriately to promote empowerment, self-management, and health education for patients.
• To attend clinical case conferences, review and clinical meetings as required.
• To be a source of clinical expertise to less experienced staff within the team.
• To delegate duties/tasks to other members of staff where appropriate.
• Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relative, and carers.
• To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure.

To ensure that patient records and documentation are maintained in line with recognised standards and ABC policies and use of the electronic patient records system. This includes:
