Position: Occupational Therapist – Social Care (Community)

Liquid Personnel are currently recruiting for an experienced Occupational Therapist to join a Social Care team in Stockton-on-Tees.

This is an excellent opportunity for an OT with community and social care experience who is confident managing a busy caseload and delivering high-quality assessments to support adults with a wide range of disabilities.

The successful candidate will work autonomously across community settings, helping individuals maximise their independence through assessment, rehabilitation, equipment provision and practical interventions.

You will have the flexibility to work from home between community visits and/or from council offices.

Key Responsibilities
  • Manage a varied community caseload independently
  • Complete holistic Occupational Therapy assessments
  • Carry out moving and handling risk assessments and planning, including single-handed care approaches
  • Assess posture and recommend appropriate seating solutions
  • Complete complex Disabled Facilities Grant (DFG) assessments
  • Provide housing recommendations and adaptation advice
  • Assess Blue Badge eligibility
  • Prescribe equipment and recommend adaptations to support independence
  • Work collaboratively with multidisciplinary teams (MDT)
  • Apply positive risk-taking approaches and proportionality within OT practice
  • Maintain accurate records using Liquid Logic care records system and ELMS equipment portal
About You

The ideal candidate will have:

  • HCPC registration as an Occupational Therapist
  • Experience working within Social Care or community OT settings
  • Experience managing an autonomous caseload
  • Strong clinical reasoning and assessment skills
  • Confidence working independently across community environments
  • Excellent communication and MDT working skills
Desirable experience:
  • Liquid Logic care records system
  • ELMS equipment portal
  • Complex DFG work
  • Housing adaptations
  • Seating and posture assessments
