The Role

Are you passionate about delivering high-quality, person-centred care in innovative healthcare settings? East Dunbartonshire has launched a new Hospital at Home Service and is seeking enthusiastic and dedicated Advanced Nurse Practitioners (ANPs) to join a values-driven multidisciplinary team.

As part of NHS Greater Glasgow and Clyde’s Transforming Together agenda, the Hospital at Home Service will provide hospital-level care to patients living with frailty in their own homes, including care homes. The service aims to reduce avoidable hospital admissions, support earlier discharge and improve outcomes for patients across the Health and Social Care Partnership.

This is an exciting opportunity to be at the forefront of transforming healthcare delivery in East Dunbartonshire, contributing to service development and supporting the delivery of safe, effective and person-centred care within the community.

All roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Duration, Location, and Working Pattern:
  • This is a permanent full-time post (36)
  • The post is based within East Dunbartonshire Hospital at Home Service Milngavie clinic G62 7AA
    - Requirement to travel between 3 localities in East Dunbartonshire
  • The shift pattern for this post is 4 days over 7, between 8 am and 8 pm, 365 days per year.
Key Responsibilities

You will use advanced assessment, clinical reasoning, interpretation and decision-making skills to assess patients, establish working diagnoses and develop appropriate treatment plans in collaboration with the responsible medical officer and wider multidisciplinary team.

You will support the delivery of hospital-level diagnostics and interventions within patients’ homes, ensuring care is delivered safely and effectively. You will play a key role in ongoing patient assessment, clinical decision-making and evaluation of care as part of the wider multidisciplinary team. You will also lead discharge planning and work closely with local community services to ensure a coordinated approach throughout the Hospital at Home admission and support safe transitions of care.

Knowledge,

training, qualifications and/or experience required to do the job:
  • Demonstrate competence in clinical assessment and diagnostic reasoning.
  • Strong clinical judgement and decision-making skills.
  • Knowledge and understanding of anatomy and pathophysiology.
  • Non-medical prescribing qualification (V300).
  • Experience of leadership in care delivery and evaluation.
  • Practice-based learning experience or equivalent transferable modules.
  • The successful candidate will have access to structured Continuing Professional Development (CPD) sessions and simulation-based learning to support ongoing training and development needs.
