Health Care Support Worker - AHP Long Term Conditions Service

NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The Long-term Conditions service is an AHP-led, digital-first self-management service. Patients referred to the team have been diagnosed with a long-term health condition which has impacted their quality of life and ability to work. Common conditions could include ME/CFS, fibromyalgia, post-viral fatigue and Long Covid, etc.

Care is provided via a 6-week virtual self-management group, specialised workshops and targeted 1-1 interventions (often via near me). This is a hybrid role; therefore, work will be undertaken remotely from home, through community home visits, and by attending one of our remote working bases (Rowanpark or Belmont) or local health centres, as required. The Long-term Health Conditions Service works across 6

HSCPs within NHSGGC boundaries. A driving licence and access to transport to support visits across this geography are needed.

All staff within the Services must comply with NHSGGC Policies. In addition to this, qualified staff will comply with the Chartered Society of Physiotherapy (CSP) or Royal College of Occupational Therapists (RCOT) Professional Standards and Code of Conduct and HCPC Standards of Proficiency.

Health Care Support Workers are required to comply with the NHSGGC Mandatory Induction Standards and Code of Conduct

This role does not meet the eligibility criteria for a Skilled Worker visa or a Health and Care Worker visa. NHS Greater Glasgow and Clyde (NHSGGC) is unable to provide candidates or employees with a Certificate of Sponsorship (COS).

Base location - Hybrid/ touchdown space at Rowan Business Park

• Works with patients individually (virtually, in their home or in an agreed setting), delivering delegated supported self-management treatment and rehabilitation programmes.
• Adjusts treatment plans within agreed parameters and reports any unexpected changes in progress.
• Supports patients with a wide range of needs, including complex fatigue, reduced quality of life, pain, breathlessness, etc.
• Will support the provision and organisation of the virtual 6-week self-management group.
• Will work with an individual to develop a wellness recovery action plan (WRAP) to support self-management on discharge from the service.
• May perform manual handling and use mobility/therapy equipment safely and competently.
• Implements therapeutic activities such as memory strategies, relaxation, anxiety management, fatigue management, and other self‑management techniques.

• Carries out delegated clinical tasks
• Uses interdisciplinary assessment tools
• Coordinates interdisciplinary interventions from assessment through to discharge under supervision.
• Provides health promotion information within a set framework.

• Liaises with multidisciplinary team members and attends team meetings to provide updates on patient progress.
• Completes referrals to other services and maintains communication with external agencies.
• Provides advice and training to patients and carers on lifestyle changes, environmental adaptations, and the impact of disability.

• Maintains accurate patient records, documentation, and statistical data (including electronic systems).
