Counterweight Limited is seeking a Community Health and Wellbeing Worker Level 3 Apprentice to support the end-to-end GP referral process and patient onboarding. You will conduct Individual Assessments, coordinate coaching sessions, and ensure adherence through motivational interviewing techniques.
You will engage with referrers, manage referrals, and provide hands-on coaching and customer support across multiple channels, contributing to a high-quality patient experience and programme delivery.
