Home-Based Dietitian

Position: Home-Based Dietitian at VHA
Deliver personalized nutrition care as a Home-Based Dietitian with VHA. Assess and create individualized dietary plans to enhance client health independently.

Join VHA Home Health Care, where you will work in a collaborative environment, providing consultation and dietary recommendations for diverse clients, including adults and children. You will ensure effective communication with the care team, addressing issues and promoting independence through tailored dietary interventions.

Key Responsibilities:

• Prepare detailed client reports for health professionals

• Create comprehensive dietary plans and presentations for clients

• Educate teams on healthy eating practices

• Support interventions for various nutrition-related conditions

• Assist clients in managing dietary restrictions and preferences

Requirements:

• Member in good standing with the College of Dieticians of Ontario

• Minimum 2 years of clinical experience preferred

• Valid Driver’s License and access to a vehicle

• Competency in electronic documentation

• Ability to provide virtual care and work independently
