The Role

An exciting opportunity has arisen for a Senior Pharmacist, Primary Care to join the Primary Care Pharmacy Team within Inverclyde Health and Social Care Partnership. This patient-facing role involves working closely with GPs and multidisciplinary primary care teams to improve medicines management, support safe and cost-effective prescribing, and deliver excellent patient care.

You will support the delivery of the pharmacotherapy service as part of the GP practice GMS contract, undertake medication reviews, develop your own clinical caseload in line with the Achieving Excellence strategy, and contribute to improving the safety and effectiveness of medicines use. Applicants should be qualified Independent Prescribers or be willing to work towards this qualification.

Duration, Location, and Working Pattern
  • This post is permanent, working part-time hours 18 hours per week over three out of five days.
  • The base for this post is Port Glasgow Health Centre, PA14 5EW
    , although you will be required to travel between GP practices, care homes and other healthcare settings across the Health and Social Care Partnership to carry out the duties of the role.
  • Please note the salary is Pro Rata for part-time hours.
Key Responsibilities

You will work as a member of the NHS Greater Glasgow and Clyde Primary Care Pharmacy Team, supporting the delivery of high-quality, safe, cost-effective and evidence-based prescribing across the Health and Social Care Partnership. You will work collaboratively with GPs, practice staff, community pharmacists, care home staff and wider multidisciplinary teams to optimise medicines management, undertake medication reviews and polypharmacy clinics, prescribe where appropriate, and provide individualised patient care.

You will also contribute to prescribing management initiatives, support joint projects across the HSCP, promote best practice in medicines optimisation, and help improve patient outcomes through the safe and effective use of medicines.

Knowledge, training, qualifications and/or experience required to do the job
  • Master's Degree in Pharmacy (MPharm) or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Independent Prescriber qualification or commitment to achieving this qualification.
  • Royal Pharmaceutical Society post-registration foundation credential, or equivalent experience.
  • Significant post-registration experience.
  • Sound clinical knowledge.
  • Good IT skills.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Experience of working directly with primary care teams and knowledge of GP systems is advantageous.
