Senior CMHACS Practitioner - Community Mental Health Acute Care Service (WD CMHACS

The Community Mental Health Acute Care Service is looking to recruit a Band 6 practitioner.

As part a multi-disciplinary/agency Community Mental Health Acute Care Service within the whole system of Community Mental Health Services the post holder will work closely with all disciplines/agencies. The post holder will provide a service for individuals with complex mental health, physical and social care needs who present in crisis and who may require intensive home treatment/care intervention, as an alternative to hospital admission.

In addition to this and to facilitate early discharge of individuals in hospital the post holder will be expected to be closely involved in discharge planning and immediate follow up from acute in service user care. This may also include assessment whilst on pass from acute in service user care.

The post-holder is responsible for the clinical leadership of more junior members of the team, prioritising workload and utilising resources appropriately to ensure all service user needs are met. However they will be accountable to the Community Mental Health Acute Care Service Leader for the management of their workload and the delegated functions to other team members ensuring that the right level of competency of their team members is matched to duties being undertaken.

The post holder would be expected to make these judgements relating to more junior members of the team through regular appraisal, clinical supervision and support including caseload management systems.

The post-holder will practice within and ensure that the practice of junior team members is within legal and ethical frameworks according to appropriate codes of professional conduct, mental health legislation and contractual obligations.

Due to the rapid response nature of the crisis model of service, the post-holder will assist the Team Leader in the rapid deployment of crisis resources in response to service user need.

The post-holder will be expected to interface between the Community Mental Health Team and In Service user services, and other mental Health Network services and agencies.

All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

• Location based in Riverview Resource Centre
• Part-time position (21.6 hours per week)
• Permanent Contract
• Shift pattern for this post are mixed: 09:00am - 17:00pm and 12:00-20:00pm shifts over 7 days

Please note the salary is Pro Rata for part-time hours.

Key responsibilities:

The post-holder will be responsible for:

• Assessment, planning, intervention and evaluation of care.
• Ensuring referrals are recorded and actioned within timescales set out in operational standards. He/She will be a resource for referrers advising on criteria for referral to the team.
• The management of their own workload. Identifying and responding to the health promotion needs of service users under his/her care.
• Establish and participate in systems of practice support and caseload supervision for junior staff.
• Identifying and responding to the health promotion needs of service users under his/her care.
• Liaison with professional and voluntary agencies to provide specialist Psychiatric advice, arranging and contributing to multidisciplinary/agency meetings and case conferences.
• Initiating and developing new ideas to improve service user care/service delivery.
• Have a sound knowledge of mental health legislation and act as an advocate and information resource for individuals and carers.
• Identifying and responding to the needs of individuals, which maybe related to gender, health, domestic abuse, child protection issues and has a responsibility to act on concerns.
• Ensure the team maintains accurate and comprehensive written and electronic records.
• Provide written reports for the service as required.
• He/She will be involved in the ongoing evaluation and review of service.
• Responsible for ongoing audit of service user/carer satisfaction with the service.
• Responsible to ensure a meaningful learning experience for students, supporting and mentoring students and meeting defined standards for clinical practice placements.
• Responsible for continuous improvement in an individuals care, utilising research evidence. He/She must access, review and critically appraise current research to ensure therapeutic interventions have a sound evidence base.
