The role involves using digital health platforms to enhance the delivery of care.

Key Responsibilities:

• Assess clients for personalized OT care strategies

• Participate in interdisciplinary team collaborative efforts

• Deliver holistic health coaching and self-management support

• Document and coordinate care through digital platforms

Requirements:

• Registration with the College of Occupational Therapists of Ontario

• Commitment to empathetic, family-centered care

• Strong collaboration and independent working skills

• Ability to apply critical thinking skills in client situations
