Pay Range Estimated annual earning potential: $105,000 - $120,000
We are seeking Physiotherapist (PT) - House Calls Program (35 hours per week)
House Calls is a Toronto-based interdisciplinary mobile healthcare team that provides care to seniors living at home with cognitive, physical and social frailties. House Calls delivers comprehensive in-home healthcare to vulnerable seniors who experience barriers accessing traditional office or clinic-based care.
A partnership between primary care, SPRINT Senior Care and VHA Home Health Care, our integrated team includes Physicians, Nurse Practitioners, Social Workers, Occupational Therapists, and Physiotherapists, supported by in-office Coordinators.
The House Calls team strives to improve the health and quality of life for all our patients. Our care plans include comprehensive and ongoing primary care, chronic and acute disease management, medication administration, in-home interdisciplinary assessments, ongoing case management, and assistance with navigating the healthcare system. The House Calls team also facilitates safe discharges and transitions from the emergency department and hospital inpatient beds to enable safe recovery and care in the home setting.

The physiotherapist will work within a continuous quality improvement framework that reflects VHA’s vision and mission of services. Key Areas of AccountabilityVHA's Physiotherapist have the following key areas of accountability, but not limited to:
Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.Implements client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
Ensures that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
Attends client related meetings to report on assessment findings and discuss plans for intervention, progress reports and discharge as required.
Ensures coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
Documents all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies.
Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the Ontario College of Physiotherapists.
Required Qualifications:
Masters degree in Physiotherapy from a recognized school of physiotherapy.
Registered, in good standing, with the College of Physiotherapists of Ontario.
