We are currently looking for Band 5 Physiotherapists to join our Community based rotations.

This post will provide a great opportunity to gain varied clinical experience, working across the Home 1st Therapy pathways:
Admission avoidance and supported discharge 2-hour response, Integrated Discharge Hub, Pulmonary Rehabilitation, Staying Steady Falls Prevention, Intermediate Care Community Therapy and Inpatient rehabilitation.

Home 1st is a community-based rehabilitation service focused on promoting independence, preventing avoidable hospital admissions, and supporting timely discharge. We work across patients' homes, inpatient units, hospitals, and community settings. As a Band 5 Therapist, you will manage your own caseload and deliver evidence-based, personalised rehabilitation. You'll work with people with a wide range of conditions, including post-surgery recovery, post-critical care needs, frailty and falls, stroke and neurological conditions, COPD and respiratory issues, dementia, musculoskeletal conditions, and palliative care needs.

You will assess, plan, and deliver individualised treatment programmes, working autonomously with support from experienced clinicians. You'll contribute to a high-quality, efficient therapy service and uphold CNWL values of compassion, respect, empowerment, and partnership.

We can also offer coaching and opportunities for daily debriefs, access to remote working, regular 1:1 formal and informal supervision, peer groupCPD sessions and in-service training, QI project work and volunteering for champion roles. • To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, CSP and HCPC code of ethics.
• To undertake assessment of clients with diverse needs through subjective questioning, objective assessment and other written material available.
• To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social emotional and mental health issues.
• To set goals for treatment with clients and/or their carer and ensure that care is client centred and agreed by the client and/or their carer.
• To set and maintain a high standard of clinical practice which is evidence based where possible, using standardised assessment and outcome measures, where appropriate.
• To screen and assess with support, complex multi-disciplinary referrals from a variety of sources. This screening requires individual responsible action when at times no other professional is currently involved. This screening will establish priority, determine needs and initiate a plan of intervention.
