The Central Norfolk Home First Hub invites applications from experienced Occupational Therapists or Physiotherapists to join its Norwich-based therapy teams. The role covers discharge to assess pathways, home visits, and collaboration with the MDT to deliver timely, patient-centred care.
You will triage referrals, assess needs, plan care packages, and work across a 7 day week to support community patients. Full HCPC registration required.
