Position: Home Health Aide at NITHA
Provide vital home-based support as a Home Health Aide with Northern Inter-Tribal Health Authority in Prince Albert, SK. Engage in health promotion, system navigation, and contribute to community outreach efforts.

As a Home Health Aide/Community Health Representative, you will focus on personal care and daily living supports.

Your role includes conducting home visits, vital checks, and accompanying clients to appointments. Serve as a crucial link between community members and healthcare providers, enhancing access to health education and services.

Key Responsibilities:

• Conduct home visits and vital checks for clients

• Navigate and advocate for client appointments

• Facilitate health promotion and distribute materials

• Support community outreach initiatives and programs

Requirements:

• Grade 12 diploma or GED is required

• Training in Community Health Representative or Home Health Aide

• Knowledge of community strengths and services

• Experience in home health support settings

• Ability to work in various client environments
