Location: Swindon
Swindon Borough Council is recruiting a Social Care Practitioner to join the Mental Health Team in Adult Social Care. You will demonstrate excellent communication and assessment skills and work in a fast-paced but supportive environment.
You will perform Care Act assessments, proportionate support planning and S117 needs assessments, following the care management process while commuting to Swindon and wider borough locations.
