Position: Winnipeg Case Coordinator - Hybrid Role
Join the Winnipeg Regional Health Authority as a Case Coordinator in a hybrid capacity. Assess care requirements and coordinate vital services for elderly individuals in need.

This position at the WRHA Community Health Services focuses on coordination and assessment of care for elderly and infirm adults. You will evaluate client needs and communicate with involved facilities and agencies, ensuring comprehensive service delivery as part of the Homecare Downtown East team.

Key Responsibilities:

• Evaluate referrals and conduct client assessments

• Establish care priorities and appropriate service channels

• Collaborate with various health service professionals

• Create individualized care plans based on client evaluations

• Monitor and assess service effectiveness and delivery

Requirements:

• Minimum two years community health experience preferred

• Relevant health/human services degree required

• Valid driver’s license and access to a vehicle

• Strong oral and written communication capabilities

• Previous experience in case coordination desirable
