This is a permanent, full-time role with part-time consideration, offering a developmental rotation across our Community Rehabilitation Hospital, Community Therapy Team, and Hospital at Home.
You will support patients to maximise independence, prevent unnecessary admissions, and facilitate safe discharge and home-based rehabilitation within their communities.
