Hampshire and Isle of Wight NHS Foundation Trust in Lymington seeks an experienced Registered Nurse or Allied Health Professional to join our Hospital@Home Team. Based in Lymington with travel across the service area, the role delivers urgent, holistic care to people living with frailty in their own homes.
You will work collaboratively with GP practices and social care partners to coordinate comprehensive home-based care. The service operates seven days a week, 08:00–20:00, with weekend rotation.
