The post holder will be central in project work for a set period of time, focusing on Frailty work and community hub work, both under the umbrella of Living With and Beyond Cancer (LWBC).

The first workstream will require focused work, contacting and working with identified patients at a set frequency for a defined amount of time. The second, will require working with key stakeholders to build the hub provision within the community for our patient cohort.

Working Pattern: 37.5 hours
- Monday
- Friday (fixed term). Some remote working required to support leave. Traveling will be part for this role.

This is an internal vacancy for Royal Devon employees. This is a fixed term contract until 31st July 2027. For current NHS employees this post could be a secondment opportunity.
