Position: 256164 Occupational Therapist

NHS Dumfries & Galloway is seeking a motivated and compassionate Community Occupational Therapist to join our Stewartry Home Team. This is an exciting opportunity to support individuals to maximise their independence, improve their quality of life, and remain safely in their own homes and communities.

As a Community Occupational Therapist, you will work with adults experiencing a range of physical and cognitive health challenges. You will assess functional needs, develop person-centred intervention plans, and collaborate closely with patients, families, carers, colleagues within the Home Team and partner agencies.

Key responsibilities include:
  • Contributing to Discharge to Assess pathways, ensuring timely hospital discharges and ongoing assessment and support within community settings.
  • Providing elements of a Rapid Response service to prevent unnecessary hospital admissions and respond promptly to changes in patients' functional abilities and care needs.
  • Assessing and addressing occupational performance, mobility, equipment, adaptation, and environmental needs.
  • Working collaboratively with patients, families, carers, health and social care professionals, and third-sector partners to develop coordinated care plans.
  • Promoting independence, choice, dignity, and quality of life through personalised interventions approaches.
  • Supporting anticipatory care planning and admission avoidance initiatives across community services.
What We're Looking For

Essential requirements:

  • Degree or Diploma in Occupational Therapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Excellent assessment, communication, and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Full UK driving licence/able to travel around the region independently (outwith public transport routes/times) and access to transport (where required).
