Community Support Worker (Career Grade) – Cognitive and Physical Disability, North West Leicestershire

Salary:£29,550 - £35,493 per annum (pro rata for part-time)

About the Role

As a Community Support Worker, you will work directly with adults, their families and carers, building positive relationships through meaningful conversations that help you understand what matters most to them.

You will undertake Care Act 2014 assessments, identifying strengths, personal outcomes and areas where support may be needed. Where eligible needs are identified, you will work with the person to develop a support plan that promotes independence, wellbeing and choice. This may include connecting people with community resources, arranging statutory services, or referring to partner agencies and support networks.

You will also play an important role in identifying and managing risk. This includes recognising safeguarding concerns, undertaking Mental Capacity Assessments and Best Interests decision making, and responding appropriately where a person's safety, wellbeing or liberty may be at risk.

This is a hybrid role, with a combination of office based working, home working and visits within the community. You will meet people in a variety of settings including their own homes, hospitals, care homes and other locations where they feel comfortable and able to participate in conversations about their care and support needs.

About the Team

The Cognitive and Physical Disabilities (C&PD) Team supports adults with physical disabilities and older people with eligible care and support needs arising from frailty, cognitive impairment and long-term health conditions. This includes, but is not limited to:

  • Dementia
  • Stroke
  • Parkinson's disease
  • Acquired brain injury
  • Other long-term neurological and physical health conditions

Our focus is on promoting independence, maximising strengths and supporting people to achieve outcomes that improve their wellbeing and quality of life.

You will work as part of a locality-based multidisciplinary team alongside Social Workers, Community Support Workers and other professionals. You will be expected to work collaboratively with colleagues across partner organisations to support people to achieve their outcomes and maintain their independence.
