Remote Rehabilitation Specialist at Lifemark

This position requires a background in rehabilitation or social sciences, along with a minimum of three years of experience in various rehabilitation services. You will assess participants' needs, craft tailored rehabilitation plans, and collaborate with an interdisciplinary team, including medical professionals and Veterans Affairs Canada. Your approach will focus on overcoming barriers and facilitating the transition to civilian life.

Key Responsibilities:

• Evaluate rehabilitation needs through assessments

• Develop personalized Rehabilitation Plans in consultation with stakeholders

• Act as a liaison among various service providers

• Identify challenges and devise strategic solutions

• Empower participants by leveraging community resources

Requirements:

• Bachelor’s degree in a relevant field

• Minimum 3 years rehabilitation experience

• Health professional designation or CVRP

• Collaborative team working background

• Ability to travel as required
